When I bought FFXI in 2003, followed by it consuming my life for the next 9 years.
A few months back after starting up again, i joined a Royal Jelly BCNM shout.
Come my turn in the BCNM, second round for the three of us, I had Refresh drop. I was excited, kinda as I had the worst drops out of our combined 6 runs (One guy got f'ing Ni and Phalanx in one run, what an freakin joke). I lot all my items and I wait for the " XXXX obtaines item". I got the complete opposite message when it came to the Refresh scroll. Refresh is lost in the FFXI world of nothingness. I ponder for a moment and realize I still have the Refresh scroll I BOUGHT off the AH(300k) but could not learn it being my RDM was not of lvl to learn it. I was lvl 40 RDM at the time of all this.
I was pretty pissed at myself and felt really stupid. Oh well, **** happens right?

I once told someone I couldn't Provoke because I was silenced. Turns out I was trying to use it before the cooldown finished.

I think I can top these. Mine is after I'd come back from a break and started levelling Scholar. I took a short break to farm up the gil for my Raise III scroll (I'm a completionist when it comes to spells). Going price was a million at the time. I started at 900k and worked up in 10k increments to try and get a good price. Finally, at about 980k, I won it. Then I tried to use it and it said I already knew the spell.
Took me a minute, but I eventually realised I'd moved the cursor by accident and brought a Regen scroll for 980k :) Bet whoever listed that on the AH got a nice surprise in their mog house the next day!

Back when we used to get the manual, I skimmed through it really quickly to get the basics, or so I thought. I was interested in being a Black Mage at the time and figured I'd try out an Elvaan. (strike 1!)
As sad as it sounds it took me until level 3 when I figured out I wasn't learning spells, checked the inventory, and saw the scroll of stone and used it. (strike 2!) However, as embarrassing as that is it gets worse.
You see, I never read the part about how medding works (FFXI was literally the first MMO I'd ever played). Through trial and error when trying to look for a healer I stumbled upon the fact that returning to the mog house heals you.
.....so, I soloed to level 9, casting Stone until I ran out of MP, beat the mob to death with the level 1 staff, and then ran back to the mog house to heal. It wasn't until I was close to level 10 when a friend that had started it up actually explained to me how /heal worked.

I've played since 2004 and had countless stupid noob moments, but I'll give one dumb thing (or a series of dumb things) I did just a couple weeks ago. Proof that experience does not eliminate completely boneheaded play.
The scene: Dynamis-Beaucedine, out farming exp for some Relic+2 enhancement. On my way back from the looooong run getting the TEs near Fei Yin, I see an Orc NM. Me: "Hey, maybe I'll get a 100!", <fights Orc NM, completely forgetting that they use a charm move>, <gets charmed and wanders around following the Orc wasting like 5 minutes while it regens to full health>.
Then I realize I'm on PUP and my automaton can easily take the Orc and won't get charmed. OK, cool. I send my automaton at the NM and hang back... but not far enough back, because I get caught with Charmga move AGAIN. Waste another several minutes following an Orc.
Take 3. I send the automaton in, decide I REALLY need to give it room this time and just leave the puppet on the Orc while I run to my farming spot. Puppet will come find me eventually. I go waaaay across the zone toward the Nue tower and my last TE (the 20min one), and start farming on the fomor near that tower. I'm having a fine time farming for a good 10+ minutes with my WHM duo partner, ignoring the fact that my puppet is busy elsewhere.
As I'm getting into my farming rhythm, I see someing weird. "Hmmm, why are there Orcs up here? I thought there were just fomor in this area." Sudden realization dawns on me that OH MY GOD THERE'S A TRAIN OF 30 ORCS THAT MY PUPPET JUST LINKED AND BROUGHT BAC <dies as the Orc alliance descends on me>.
Yeah, I'm a 10 year veteran player who has played PUP since ToAU launch... /facepalm
