Mr. Pinchy is only fished out of Highland Mixed Schools in Terrokar Forest.
There are a total of 17 Highland mixed schools in the three Fly-to only lakes in Terrokar Forest. 3 in the lake above Stonebreaker point, 4 that spawn in the lake just above and behind Allerian Stronghold (Lake Ere'Noru), and 10 in Blackwind Lake/Skettis area.
Mr. Pinchy's drop rate appears to be about a 0.2% drop rate on successful casts (about 2 out of 1000 or 1 in 500 casts) so be prepared to spend alot of time on this.
Upon Catching you will get one of the following results:
1. Mr. Pinchy's Gift, contains 4-5 Super Healing, and 4-5 Super Mana potions
2 - Mr. Pinchy's Blessing, 2-hour 1200hp buff that persists through death!
3. Furious Mr. Pinchy, Lvl 70 non elite that attacks you and your party, is not lootable.
4. Benevolent Mr. Pinchy, Lvl 70 Guardian that assists you for 10 minutes
5. Magical Crawdad Box, summons a Magical Crawdad non-combat pet, very rare.

Was farming Spicy Crawdad for my partner for his warrior.... Had caught about 30 crawdads and 20 goldfish.
I have Nat Pagles pole from Hinterlands (horde quest), with silver hook lure (daily fishing quest) and the beer that lasts 3mins and gives +fishing buff.
I was AMAZED!!!!! to catch him!
And I got Magical Crawdad pet on 1st try. Cant wait to see wot I get in 2 days :P

You will need 420 fishing to Cast into the Highland Mixed schools, and about 480 to loot without many "Your fish got away!" messages.
You will no longer receive "Your fish got away!" messages if your fishing skill is 525 fishing or higher.
I had the games max limit of 527 Fishing the 3 times that I have caught him.
375 Base Fishing Skill
+40 Arcanite Fishing Pole (I have the High Test Eternium Fishing Line on it so 35+5=40)
+5 Lucky Fishing Hat
+5 Nat Pagle's Extreme Angling Boots
+2 Fishing Skill enchant to gloves

considering such a low drop rate on Mr. Pinchy to begin with, and then only 3 chances to get this box. Otherwise, time to try to get Mr. Pinchy again for another 3 chances!
Edit: Got my first Mr. Pinchy today, hoping to see the pet. Used it and got the the guardian. I'm currently fishing with my Seth's Graphite Pole (+20 skill) and Sharp Fishing Hooks (+100 Lure), drunk from time to time on Captain Rumsey's Lager (+10 skill for 3 minutes). I have base skill of 375, so I'm getting 495-505.

But now as my priest, buffed up 522 skill, I get the message around every 3rd pool of highland mixed.. But looking still forward to get the damn crab..
Edit: actually, I dont get that message anymore.. (or very very rarely)
EDIT: Maybe I shouldnt write this now as I ve been awake way too long and drinked too much coffee.. Hehe, but Im laughing so much for the though that I just had to share it with you right now.
Well, as I fish in skettis place I get often attacked by water elementals and as having all that +fishing gear on me, my +spell damage is just around 400.
Well, having fishing pole as melee weapon dont bother me so much as I cast spells, but think about a rogues.. haha!! What they do, they spank 70lvl elementals with a fishing pole.. Basic 17,5dps on Arcanite fishing pole.. plus their +ap which is decreased by using fishing gear, they must enjoy that a lot.
I wish luck all you by trying to catch Mr. Pinchy.

First wish was Magical Crawdad Box and second wish (today) was another Magical Crawdad Box! So now I have 2 in my inventory... Getting it twice is very possible!
I was suprised when I saw the arcane explosion animation again and received another one... wishing it wasn't BoP so I could sell it lol...
2 more days till my 3rd wish, lets hope I get Mr. Pinchy's Gift :D

Got my 4th pinchy today but managed to press a movement button so it got lost.

What Ive seen so far is that the droprate is about 1 %

I was so happy... The blue picture popped up and autoloot asked me to confirm the looting of this item... Before I could say YES! my levitate wore off and I fell into the lake cancelling my loot window and setting Mr. Pinchy free. I was very very sad... no Mr. Pinchy for me... I hopped up levitated again and cast another line in the same pool... There he is! Mr. Pinchy is back! Not a particularly interesting story but I guess a pool is "destined" to have a Mr. Pinchy in it when it is born and Mr. Pinchy will stay there until someone grabs him up.
I've only used it once and got Benevolent Mr. Pinchy. He does melee damage in the ballpark of 225 with a good crit rate. He can be buffed and healed. I hope he hands out the Non-combat pet next time cause it is something money can't buy.

i agree with Ninjakicks theory of pre-destination after that ^.^
Aside from the pools having furious crawdads and golden darters ( my guild loves me now for passing out food during boss fights on raids ) I was lucky enough to pull up Mr Pinchy jsut as my last Aquadynamic Fish Attractor was expiring.
First use received the non combat pet. Second use Furious Mr Pinchy came out, and 2 days until the third one.
The non combat oet alone is worth going for this guy, and you could most likely make some decent coin selling the fish used for food as well.

I'm not sure what level fishing the pools are however they arent fishable at 341 however using +52 in equipment and the +75 baubles they are.
Use #2 (update): I waited 2 days and Mr Pinchy gave me a second "Magical Crawdad Box". Drat I was hoping for something else. Of course I'll be back and post what I get for my 3rd use in a few days.
Use #3 (update): The little bugger has forsaken me once agan and granted me another magical crawdad box. Looks like I need to fish him up again.
