There are four of these in An Ethereal Rift. They scale to your level, Heroic one down-arrow.

Outsiders

While on this quest, you will see portals throughout Loping Plains, Everfrost, Lavastorm and Zek, the Orcish Wastes. Standing near the portals for a few seconds will spawn an extraplanar entity, which is a straight con mob that scales to your level.
