EQ2 Quest:McBlain Who  

This quest is initiated by examining McBlain's Journal, a small book in the water at a rock near the entrance of Lavastorm ( -143, -131, 610 Copy ).

You must slay 15 Magma Wormlings in Lavastorm. After doing this you will receive McBlain's Dagger.

You can examine this item to initiate the McBlain's Broken Dagger quest.
