If anyone else has the issue where the enemy portrait/HP/mana would disappear mid fight (you would keep them targeted the frame would just not be there) the problem, for me at least, ended up being CTRaid.
Just an FYI, I know a few people were having this problem.
I haven't had a chance to look for an update yet, so not sure if an update fixes it.

I don't use CTRA and I was having the same issue. It happened while fighting Onyxia on my priest, and while out solo on my hunter.

I too was noticing this.
Yeah, this caused me to die a few times in BG (no enemy portrait = no enemy castbar).
I don't think I've had this problem myself but I've been hearing about it all day long.
Beats me why it happens to some and not others :S
It's not just CTRaid. Several guildies and I were having problems with this, we tried disabling addons, some even did full, clean reinstalls, and the error keeps occuring. In fact, it seems even worse with the stock blizz UI.
The one "fix" I have seen so far is to download Perl Classic. That cuts about 95% of the problems. I still have issues on my hunter or warlock with my pet bars disappearing randomly or just not showing up in the first place, but it reduces the amount that the targetting bug occurs dramatically.
What's really odd is that on my druid, I wont get a target frame, but my combo points will still show up, with me having no way to tell what they're on.
Other unresolved issues:
Raid promotions/leader changes going to random raid members.
Kicking people from raid kicks a random person.
Raid buffs not showing up properly when targetting someone outside your party.
Buff icons not displaying properly for buffs.

haha yea had that on my rogue.
For me removing CTRaid solved my issue, i had no problem with mage (using WE pet) the bar stayed up etc.
Strange indeed.
Better they did this before the expansion at least so they can do a quick hotfix hopefully before the 16th.
Just checked the Bug Report forum, and the official Blue line is that it's an addon problem and not their fault....
...I guess they didn't read the pages of posts by people who have no addons at all and can still replicate the bug.
With all the errors that are currently in game, I'm really dreading BC. The sheer volume of bugs is making me really worried for the expansion, if it's done at the same "quality" as this patch.

I'm having the same problem. I've heard lots of people saying that CTRaid is the culprit, but then several others who don't have that and are still experiencing it. My boyfriend is using Nurfed UI and he's having no trouble.
On the bug fix forums there was this blue post saying that this cannot be hotfixed but that it will require a new client...

I'm not using CTRaid and having the same problem. Seems that the issue revolves around being in combat - once you are in combat the UI can no longer draw new frames. This was supposed to stop mods like decursive but it looks like it has borked the UI in a big way...
