You can obtain them by:

• Completing a zone-wide death event: 5 Otherworldly Sourcestone
• Helping complete a zone death event boss: 5 Otherworldly Sourcestone
• Completing a major death rift: 1 - 5 Otherworldly Sourcestone
• Completing a minor death rift: 0 - 2 Otherworldly Sourcestone
• Completing World Event Daily Quests: 15 Otherworldly Sourcestone
