It's been reported this quest is actually called "Diary of a Madman". We need confirmation. Or are there two book quests? I remember Holdout on Beta.

All the information in Zam listed for the "Holdout" quest is correct *except* where the quest originates. The co-ordinates that Zam lists as the location on where to begin the quest are actually the coordinates of where to start the "Diary of a Madman" quest. If that were not confusing enough, the location Wiki lists for "Diary of a Madman", are actually the location on where to start "Holdout".

The Quest "Diary of a Madman" is offered by reading a ground spawn page inside one of the tents at the lzardman camp at 167, -9, 164.
The Quest "Holdout" is offered by reading the book in Oggok at -442, 5 135.

All updates are found in the City of Oggok.
• Page 1: -302, 7, 225
• Page 3: -461, 5, 230
• Page 4: uncommon auto-update from Zek servitors or devotees
• Page 5: -358, 3, 309
• Page 6: uncommon auto-update from Zek servitors or devotees
• Page 7: -342, 5, 162
The quest will auto-complete when you find all the pages.
However:
The quest seems to be buggy. I was able to get the Mob spawned pages, as listed above, but could find NONE of the ground spawn pages at all!
