I have been trying to get one of these for the longest time on my other character. I have done numerous campaigns (although being highest at 45 doesn't yield much and ends up in death most of the time). Also have completed the required quests and WOTG missions up to 3-1. Also, I have accumulated a whopping 3400 in notes through Operations. Still I am denied advancement. Was wondering if anyone knew exactly what the requirement is to obtain it. Much appreciated.

According to the wiki you need to get 3000 Allied notes from campaign battles. Here is a link that contains a chart for the different campaign ranks Campaign FFXI wiki page

Unfortunately from what you say is that you have over enough allied tags to get a rank up. Wish I could give a little more advice about that.
