I think it gets a bit more hate because the potential for negative digestive response increases dramatically when the low quality food you're consuming consists mostly of ground beef and refried beans. It may be cleverly shaped into various forms, and wrapped in different types of tortillas, but we're still talking about mostly ground beef and refried beans. The beans need no other explanation, but even the ground beef is a problem since it's not ground beef that comes pre-made into a patty like at a burger joint, but is cooked and sitting in a bin waiting for someone to grab a handful out and put it into said tortilla with said beans.
I'll also disagree that everything else fast food is just as bad. There are pretty noticeable levels of fast food and Taco Bell is comfortably on the bottom, along with McDonalds and Burger King (and pretty much any nationwide chain). Mid tiers tend to include more regional chains (although some have become national over time) like Carl's Jr/Hardies, Wendy's, and Jack in the Box. At the top are places like In and Out. There really is no comparison.
If it helps though, if I had no choice but McDonalds or Taco Bell, I'd choose Taco Bell every single time. I place McDonalds into a special place of horribleness all by itself. But that's just me.
