You have an industry fueled by ignorance and fear. You have many individuals that aren't cost sensitive, and we can debate endlessly whether this is because of the ignorance (Why worry, my insurance covers everything for $25!) or fear (Take my money please! I don't want to die!). It's probably both, and the field has gotten itself in a cozy position where there's little incentive to reduce either. A fear of malpractice lawsuits make doctors play it safe by ordering a few extra tests. A willingness to pay whatever it takes for a chance at a cure removes any real incentive to reduce fees.
