How did the dog die exactly? I mean you don't die from getting your head stuck in something unless he stabbed himself somewhere in the neck/head or broke his neck trying to get back out.
nvm he probably choked if the spot in the kennel was really tight that he got his head into, sorry for your loss 
I do believe he mentioned- Stiff and Cold, IE: left out to the elements
I'm assuming it broke its neck.
