The inaccuracies come from anyone else in your party.
It parses your own actions nearly perfectly, but misses a substantial amount of data when parsing others. Even if they never leave range and are fighting right alongside you.
Double parse with someone, start at the same moment, never stray from that person. You will see what I mean after only a few moments.
