Right now I'm on my level 91 PLD (Gimp, returning player!) and am curing myself with it returning 0 HP to me, even when I was in the red, had no status ailments and my spell wasn't interrupted. What's going on?

That's likely what it was. What mob were you fighting? Certain NMs like Briareus, Cottus, Kukulkan, and others have a special version of Curse that, while its active, prevents you from recovering HP and MP by any means and can only be removed by Sacrifice.
If I may ask, what belt are you using on your Paladin? If you are using the Accursed Belt, it has a hidden effect of that same special Curse, which makes it impossible for you to recover HP and MP with it equipped.

I would imagine more people when it first came out than present. That said, getting a Goading Belt shouldn't be too difficult, just look for people making Emp weapons on Chloris path and offer to help out. Can't remember if PLD can use the one Ovni drops, but I look at that one rather hesitantly with the -stats associated and knowing how easily that'll let mobs debuff you. Could even try camping KA.
